关于 JU's Insurance Requirement

所有的全职 Traditional 本科 Students enrolled for a minimum of twelve (12) credit hours 和 研究生 Students enrolled for a minimum of six (6) credit hours will automatically be enrolled in the JU Student Medical Insurance Plan 和 the premium for annual coverage is added to the tuition billing upon fall registration. Coverage can be waived if evidence of existing 类似的 coverage is presented prior to the waiver deadline.

General 规则 for Enrollment

The university student health insurance policy is compliant with Federal 健康 Care Reform law (section 1302b of the PPACA).

Students may enroll after the deadline only if there has been a significant life change (i.e. loss of prior coverage) 和 should contact the JU Student 健康 Insurance Administrator at +1 (904) 256-7080 or StudentAccounts@cmithlj.com. A written request must be submitted to Student Accounts with documentation as to why they no longer are insured, along with proof of dropped coverage from the prior 航空公司. The rate will be pro-rated based on the month the student enrolls.

如果你有 existing coverage 和 need to waive the JU 健康 Insurance policy, your coverage must be 类似的 和 meet the following requirements:

• Your plan must cover you from August 1, 2024 to July 31, 2025 for the 2024-2025 Academic Year.
• Your plan must cover 常规的 physical/mental health 和 wellness, diagnostic testing, preventative services, injury, 等. visits to a doctor in Jacksonville, FL, U.S.
• Your plan must cover 急救护理 in Jacksonville, FL, U.S.

• Attach clear images of your insurance卡 和 政策概要, clearly highlighting coverage.

Please note that undergraduate 和 graduate students have different rates, so review the documents carefully to ensure it is relevant to your status as a student. It is the student responsibility to verify that their existing coverage meets the above requirements. JU reserves the right to decline submitted waivers due to incomplete or insufficient proof of coverage.
